Important medical documentation for next WAKO Cadets & Juniors World Championships 2016

Dear WAKO members, Please find herewith attached important documents that each competitor at next Cadets & Juniors World Championships 2016 will have to fill in and deliver at the moment of accreditation and official weigh-in in Dublin. It is really important that you provide to distribute the following documentation to your medical teams and also your Coaching teams, who in turn will have to inform all athletes of these documents and procedure. Documents attached : 1. WAKO Liability Waiver Form : Must be signed by EVERY participant and official at WAKO Continental – World Championships; 2. WAKO Medical Questionnaire : Must be filled out completely and signed by every competitor and official at the morning of weight control and medical check and handed in to the doctor, before they begin the medical check or register as a referee; 3. WAKO Medical Form : This form must be signed and stamped by a qualified doctor and brought with you when presenting yourself for Medical Check and weight control. *** Special Note no group sign off is allowed, this document must be signed and stamped for every individual attending the championships ***

Medical Form is valid for 1(one) year and it must be returned to the competitor after medical exam control in Dublin. In fact MF can be used at other competitions as valid medical document.

This document relates to WAKO Continental and World Championships only, events run by WAKO IF and or its Continental or SportAccord – World Games partners. It is the responsibility of national teams to bring these documents with them, duly filled in and signed.
